Case Planner, Family Treatment and Rehabilitation

Under the supervision of the FTR Supervisor, the Case Planner will be responsible for case intake, management of cases and providing service needs, in accordance with ACS standards and indicators and Safe Space policy and procedure manual.

 

Essential Functions:

  • Evaluate clients to assess safety, risks and develop case plans and coordinate all case management, referral and advocacy services for each family.
  • Monitors and follows up on individualized treatment programs including child safety.
  • Individualizes and understands the needs of the individual client, the family and its support network in relation to each other.
  • Conduct home visits in which the needs of the family and child safety are assessed.
  • Home visits must take place weekly and may require case planner to walk up steps. Evening hours and a few Saturday coverage days may be warranted for some cases. 
  • Conduct field visits to identify services for clients and helps families maintain medical, financial, educational and social resources.
  • Identifies appropriate treatment or service providers and facilitates client referrals to internal and external and/ or community linkages.
  • Establishes client goals and assesses a client’s success in meeting those goals; continues, alters and recommends changes in treatment strategy depending upon ongoing client assessment.
  • Advocate on behalf of children and families to obtain the necessary neighborhood based services.
  • Required to complete FASP’s (Family Assessment Service Plan) every 90 day and six months.
  • Responsible timely submission of all paperwork, completion of intakes as well as progress notes and all necessary paperwork.
  • Performs other duties as delegated

 

 

Qualifications:

  • Bachelors degree or Masters Degree in Social Work
  • Good writing and oral communication skills; computer literate
  • 2-3 years experience in child welfare field strongly preferred
  • Excellent time management skills with the ability to meet deadlines in a timely manner while managing competing priorities
  • Bilingual, (preferably Spanish, French/Creole) to conduct case management for our families
  • Excellent written and oral communication skills
  • Proficient use of Microsoft applications; knowledge of PROMIS/CONNX desired
  • Cultural awareness and sensitivity when serving clients that come from different backgrounds and walks of life
