Case Planner (NSP)

Sheltering Arms is currently looking to hire a Case Planner to provide comprehensive case management services for all youth who reside in the Non-Secure Placement facility and their families. The Case Planner should be committed to a service philosophy and objectives that is youth-centered and outcome focused. The position is demanding and highly interactive with the youth and their families, staff and community.  Case Planners must be comfortable working with youth and families in the context of the facility, the office and in the community of origin.

Essential Functions

  • Responsible for development of a comprehensive service plan for each youth and the youth’s family or other adult resource. The service plan will be completed in collaboration with the team members, and with the youth and his adult resources in the Family Team Conference.
  • Work with team, the youth and the family to develop a discharge plan as part of the service plan.
  • Schedule family visits within 24 hours of admission to the agency. 
  • Engage the family members throughout the assessment process and completion of the FASP.
  • Responsible for meeting required ACS completion dates for accurate progress notes, FASPs, and visits.
  • Implements creative strategies for involving family members in the service planning process.
  • Home visits to engage and assess the family in collaboration with the Visitation Specialist.
  • Maintain efficacious working relationships with staff involved in the service planning and provision process: Youth Specialists, Mental Health Therapist, Educational Personnel, Court and Legal Representatives, Medical Personnel, etc.
  • Attend court hearings, and prepare reports as required to be submitted before and at the hearing;
  • Primarily responsible for the development, documentation and maintenance of all case records within your caseload.
  • Provide necessary referrals for families in need of housing, health, mental health or preventive services.
  • Support the principles of the Missouri Model with the objective of teaching youth to make positive, lasting changes in their behavior.
  • Lobby and advocate energetically for the appropriate educational placement for each youth.
  • Network with other agencies to stay informed about services
  • Work closely with Quality Improvement to ensure CQI (continuous quality improvement) of services rendered.
  • Other related tasks that may be assigned.

Qualifications

  • Bachelor’s Degree in Social Work or related field required
  • Strong knowledge of ACS mandates  
  • Prior experience working with foster boarding home or residential teens a plus
  • 2+ years experience with Foster Care or Preventive case management
  • Advanced knowledge of Connections required
  • Strong writing skills, including competency with progress note completion
  • Familiar with Family Court
  • Must be able to pass the Safe Crisis Management (SCM) training.

Must have the ability to complete all standard administrative and support tasks including but not limited to climbing up and down stairs, operation of computers/phones/fax/printer/copy machine, and the ability to lift boxes, furniture, and equipment up to 30lbs. Must be able to travel from the various locations and sites of Sheltering Arms. Ability to perform physical restraint of youth using approved restraint techniques. Physical and PPD clearances required and reevaluated annually.
