Job Description: Under the direction of the Director of Mental Health Services, coordinates the health and mental health care of youth in LSP facilities.

Essential Functions:

  • Participates in training, and is knowledgeable of the Integrated Treatment Model, as well as all relevant procedures and protocols relevant to staff conduct within LSP facilities.
  • Is aware of all health and mental health protocols as they relate to LSPs
  • Tracks and reviews all data and other reports of IS program to assure timely delivery to 21 day or other time frames of youth in temporary care.
  • Compiles reports on treatment data provided by all three facilities, and Clinical Supervisor of LSP
  • Works with Facility Directors and After Care Services to maintain and develop positive community relationships with relevant community boards and groups, and attends relevant meetings of same.
  • Assists in reviews of medical records upon intake and on an ongoing basis.
  • Conducts educational seminars for Aftercare staff and parents regarding community programs and services relevant to discharge goals.
  • Collects information from Aftercare, Case Planners and Education and Life Skill coaches to compile and maintain database on communities to which youth are returning, including relevant supports in all related services. 
  • Attend conferences, seminars and agency meetings as designated by the Director of Mental Services
  • Assure that all OCFS, ACS, and OSHA rules and regulations are maintained
  • Help assure that the clinic is adhering to JCAHO rules and regulations
  • Community Liaison for both LSP’s in Bronx and Queens
  • Develop a Community Advisory Board for Both Bronx and Queens

REQUIREMENTS:

  • Bachelor’s degree
  • 5 years or more of experience in clinical care or data management
  • Knowledge of New York City and State Foster Care Systems.
  • Computer literacy
  • Knowledge/Experience in records management
  • Good written and verbal communication skills 
  • The ability to exercise good judgment in emergency situations, to take initiative appropriately and to function independently when required. Insight to know when and where to get help.

Physical Requirements:

Must have the ability to complete all standard administrative and support tasks including but not limited to climbing up and down stairs, operation of computers/phones/fax/printer/copy machine, and the ability to lift patients, boxes, furniture and equipment up to 30 lbs. Must attain physical and PPD clearance to be eligible to work with our populations. Must be able to drive an automobile throughout New York City.
