Case Manager, Rapid Rehousing

About the Program: Rapid Rehousing for Young Adults is a solution to homelessness designed to help young adults aged 18-24 quickly exit homelessness to permanent housing. The program provides rental assistance and support services that are tailored to the unique needs of the young person.

 

Job Summary: Provide case management to a case load of 12-15 youth in the Rapid Rehousing Program.  

Essential Functions

  • Complete intake process with youth according to established guidelines, and make appropriate contact with and/or referrals to other agencies.
  • Complete all need assessments including but not limited to mental health and social history
  • Obtain legal documents (birth certificates, NYS identification, social security), assist with housing applications, assist with employment and enroll clients in school.
  • Develop individualized service plans for each youth and update monthly
  • Facilitate, develop and participate in skill building groups.
  • Provide bi-weekly individual and family case management sessions to each youth.
  • Make referrals for outside services when needed such as substance abuse, mental health, etc.
  • Lead intakes, treatment team meetings and discharge planning meetings.
  • Provide crisis intervention to youth in their apartments and mediate roommate issues as needed.
  • Assist and complete all required reports daily, monthly, quarterly and yearly.
  • Keep updated chart records for all current and past residents and complete all mandatory paperwork required by contracts and funding sources.
  • Work alongside all other program staff and Director to develop and evaluate programming
  • Conduct outreach and presentations as needed and as assigned
  • Attend regular staff meetings and trainings.
  • Accrue at least 40 hours of training annually.
  • Other duties that may be assigned.

Qualifications and Skills

  • A bachelor’s degree from an accredited college
  • Between 2-5 years of experience of working with adolescents required
  • Knowledge and understanding of the homeless population
  • Experience conducting groups and developing appropriate and relevant youth programming
  • Comfort traveling throughout NYC
