Job Summary

The Aftercare Education Specialist ensures that all NSP youth receive an appropriate education experience while on aftercare. The Aftercare Education Specialist will collaborate with the NSP Education Coordinator to develop a discharge plan and will be responsible for implementing that plan. The Aftercare Education Specialist will work in collaboration with DOE school staff, Aftercare Specialist and/or MST Therapist to provide supports for the youth’s education needs in the community. Aftercare Education Specialist will be trained in the Integrated Treatment Model (ITM) and in Multi Systemic Therapy to ensure continuity with the concepts the youth and family have learned while in placement.

Responsibilities and Duties

  • Collaborate with Case Planner & Education Coordinator to determine education opportunities upon discharge for youth including but not limited to: GED programs, special education r, high school referrals and vocational programs.
  • Develop a discharge education plan with the youth and the family in collaboration with the clinical and case planning NSP team and the DOE staff.
  • Lobby and advocate energetically for the appropriate educational placement for each youth in aftercare.
  • Partner with the community school in assessing the youth’s educational abilities and goals, with a specific focus on any special education areas
  • Monitor the youths’ attendance and grades, coordinate school meetings and maintain a relationship with school personnel.
  • Develop and submit monthly tracking reports of youths’ educational progress.
  • Act as a liaison between Sheltering Arms and NYC Department of Education.
  • Maintain accurate documentation and complete the timely entry of progress notes into CONNECTIONS in accordance with city, state and agency standards.
  • Maintain the Education Tab in CONNECTIONS updated while youth is in aftercare in accordance with city, state and agency standards.
  • Support the principles of the ITM Model with the end goal teaching the young people to make positive, lasting changes in their behavior by preparing them to be self-sufficient through education and work experience.

Qualifications and Skills

  • Bachelor’s Degree required, Master’s Degree preferred in Education, Social Work, School Psychology, or related field
  • Strong knowledge of ACS mandates and Connections
  • 2+ years prior experience working with residential teens
  • Strong knowledge of the specialized and alternative programs in NYC DOE
  • 2+ years of experience in educational services
  • Strong writing skills
  • Strong advocacy Skills
  • Must be able to pass Safe Crisis Management (SCM) training.
  • Must be able to travel from the various locations and sites of Sheltering Arms. Ability to perform physical restraint of youth using approved restraint techniques.
