Sheltering Arms is seeking for Case Manager for Residential Center in Harlem. New Case Manger will assist youth and families to access appropriate services and resources internally, in community or at other organizations. Provides Case Management and Counseling services to at-risk youth and their families.

Essential Functions:

  • Provides case management, counseling to prevent homelessness among at risk youth
  • Carries a caseload and presents relevant details and progress at meetings and case conferences
  • Conducts assessments and refers youth that display a need for more intense counseling or are in need of mental health services to Social Worker or mental health/clinical services.
  • Completes paperwork for youth in a timely fashion including but not limited to case notes, assessments and referral materials.
  • Works with street and homeless youth to identify options for housing, including returning home or with family if deemed appropriate
  • Acts as a liaison with any existing service providers involved with the youth or their family
  • Works with family members, when possible, and encourages them to become the young person’s primary support system
  • Provides follow-up services to youth who reunite with their families for a period of 90 days or longer
  • Facilitates workshops at the Drop-in center and encourages active participation among youth
  • Submits reports to supervisor and/or funder(s) on time and in appropriate format
  • Works alongside all other program staff and Director to develop and evaluate programming
  • Conducts outreach and presentations as needed and as assigned
  • Performs all other program and position duties as delegated by supervisor that will may also support all other Youth Services programs 


  • MSW required
  • Knowledge and understanding of at-risk youth population required
  • Experience conducting groups and developing appropriate and relevant youth programming
  • Between 1-3 years of experience working in the field of human services
  • Flexibility in schedule required

Physical Requirements: Must have the ability to complete all standard administrative and support tasks including but not limited to climbing up and down stairs, operation of computers/phones/fax/printer/copy machine, and the ability to lift boxes, furniture, and equipment up to 30lbs. Must be able to travel from the various locations and sites of Sheltering Arms. Physical and PPD clearance required.
