Job Summary: The Case Manager provides a safe, secure, non-judgmental, respectful and supportive living environment for all residents.   

Essential Functions

  • Complete intake process with clients according to established guidelines, and make appropriate contact with and/or referrals to other agencies. 
  • Ascertain that a medical health assessment, completed by the required personnel, has been scheduled within 24 hours of a resident’s admission. 
  • Complete all need assessments including but not limited to mental health and social history
  • Obtain legal documents (birth certificates, NYS identification, social security), assist with housing applications, assist with employment and enroll clients in school.
  • Facilitate, develop and participate in skill building groups.
  • Provide weekly individual, group and family case management sessions to each resident.
  • Assist providing mental health, substance abuse, trauma sensitive and all other counseling as needed.
  • Make referrals for outside services when needed such as substance abuse, mental health, etc.
  • Lead intakes, treatment team meetings and discharge planning meetings.
  • Get regular updates from full time staff and when not available read staff communications at the beginning of every shift.
  • Provide crisis intervention such as calling police, ambulance, or other emergency services.  If emergency services are called inform your supervisor and the Director within an hour of the incident.
  • Assist and complete all required reports daily, monthly, quarterly and yearly.
  • Keep updated chart records for all current and past residents and complete all mandatory paperwork required by contracts and funding sources.
  • Attend regular staff meetings and trainings.
  • Accrue at least 40 hours of training annually. 
  • Other duties that may be assigned. 


  • Master’s Degree in Social Work or related field; LMSW, LCSW or LMHC required
  • Between 1-2 years of experience of working with adolescents 
  • Knowledge and understanding of the homeless population
  • Experience conducting groups and developing appropriate and relevant youth programming

 Salary: 45k

Shift: 9am-5pm
