Sheltering Arms is looking for a Full Time Intake/Outreach Coordinator for Our Hope and Care program, in which provides case management, health education, Peer Navigation services and HIV/STD prevention outreach and education throughout Queens.  The Intake/Outreach Coordinator is responsible for completing the brief intake for all clients entering services and referring them to the appropriate staff; recruiting and training individuals to become peer HIV educators; supervising the part-time outreach staff and peer educators; and has primary responsibility for scheduling all outreach and education activities to meet program goals with a particular focus on recruiting clients into case management and groups.

Essential Functions

  • Complete brief intake for all clients entering services and refer to the appropriate intervention
  • Recruit, train and supervise Peer Educators for HIV & STI prevention interventions, street outreach, and community events
  • Supervise the part-time outreach staff
  • Facilitate groups using evidence based and evidence-informed curricula
  • Conduct outreach  (street outreach, van outreach, community presentations)
  • Facilitate HIV, STD and other health related presentations throughout the community
  • Plan for program special events (World AIDS Day, National HIV testing Day, etc.)
  • Attend and participate in supervision, staff meetings, trainings, and conferences
  • Connect with other organizations to recruit participants for case management, HIV education and prevention groups, and non- HIV related agency services
  • Distribute condoms and safe sex information
  • Perform administrative duties, including entry of client related and program data into AIRS database system, maintaining peer charts, and assisting in the implementation of peer program evaluation activities
  • Prepare monthly stats and reports
  • Maintain up-to-date linkage agreements that benefit clients in all areas of the Hope and Care program, particularly case management
  • Represent the Hope and Care Program on community coalitions and boards as needed

 

Qualifications

  • Bachelor’s degree in a related field preferred with three to five (3-5) years of experience required, one to two (1-2) years in a supervisory/leadership role
  • Group facilitation experience
  • Experience working with adolescents
  • Strong presentation and writing skills
  • Has intermediate level HIV/AIDS and STI/STD knowledge
  • Sensitive to cultural, ethnic and gender orientation differences
  • Ability to read and write English proficiently
  • Must possess great social and interpersonal skills
