Job Summary: To provide short or long term case management and parent education services to prenatal
and parenting families residing in Jamaica and Southeast Queens, with children 0-2.


Essential Functions: 
 Participate in street and community outreach, community presentations and other efforts to educate
the community about the program and increase referrals; Target hard to reach or transient families
such as families in shelters and women not connected to prenatal care.
 Obtain consent and provide prenatal and postnatal information and support to a caseload of 25-30
families on a variety of topics (breastfeeding, childbirth education, safe sleep, family health, pre-natal
and postpartum care, parent child interaction etc) through the use of groups, workshops, and case
management; Utilize the Growing Great Kids curriculum to provide support around parent child
bonding and attachment individually or in groups as needed (such as “Mommy & Me” or “Daddy &
Me” programming)
o Groups and workshops may take place off site such as in schools, WIC centers, hospitals
and hospital satellite sites, shelters, libraries depending on the needs of the population etc.
 Assess families for risk factors for pre-term birth and poor child and family outcomes using several
screening tools and determine appropriate referrals; refer high risk families to long term home
visiting/case management programs.
 Provide community referrals as needed and accompany families to health and human service
appointments as needed in order to link them to services; coordinate care and advocate for families
to receive high quality services
 Provide timely documentation of all activities, including information gathered on health and
development and family functioning; this includes data entry responsibilities using Challenger Soft
Data System.
 Organize and execute tasks at client/program activities and events, which may be held in evenings or
on weekends and may require some moderate lifting of items

 Complete all program trainings as assigned
 Establish a trusting relationship with families and remain strength based
 Other duties as assigned


Qualifications:
 Bachelor’s or Associate’s Degree preferred but the minimum requirement is a high school
diploma/GED
 Experience working with high-risk families in a community setting
 Have knowledge of infant and child development
 Ability to be non-judgmental and establish trusting relationships; good problem solving, listening and
practical skills; demonstrates maturity, self awareness and emotional well-being; shows respect and
sensitivity with individuals of differing cultural, racial, sexual or ethnic identities; some knowledge of
community resources.
 Open to working evening hours and weekends when necessary
 Fluent in Spanish required

Salary: Starting 32k
