Sheltering Arm is seeking a Case Manager to provide advocacy, referrals, supportive counseling to individuals and families infected or affected by HIV/AIDS.

 

Essential Functions

  • Provide comprehensive and supportive case management to address short term and long term needs of clients
  • Provide supportive counseling, advocacy, referrals to other Sheltering Arms programs and outside agencies
  • Maintain client files and documents including updating charts, completing intakes, progress notes, goal assessments, and service plans according to established time lines
  • Accompany clients to appointments
  • Home and hospital visits
  • Enter client related and program data into AIRS database system
  • Complete and submit monthly reports on time
  • Distribute condoms and safe sex information
  • Facilitate and Co-facilitate groups as necessary
  • Maintain up-to-date linkage agreements that benefit clients in all areas of the HIV programs, particularly case management
  • Represent the HIV programs on community coalitions and boards as needed
  • Attend and participate in supervision, staff meetings, trainings, and conferences
  • Other duties as assigned

Qualifications

  • Bachelor’s degree
  • Knowledge of services and entitlements for individuals affected or infected by HIV/AIDS
  • Group facilitation experience.
  • Must possess great social and interpersonal skills
  • Strong presentation and writing skills
  • Has intermediate level HIV/AIDS and STI/STD knowledge
  • Sensitive to cultural, ethnic and gender orientation differences
  • Ability to read and write English proficiently
  • Bi-lingual in Spanish/English preferred
