Position Summary

Care Management is a service model whereby all of an individual's caregivers communicate and interface so that the member's needs are addressed in a comprehensive manner. This is done primarily through a "care manager" who oversees and provides access to all of the services an individual needs to assure that they receive everything necessary to prevent hospitalizations, stay healthy, and maintain stability. 

 

The Senior Care Manager carries a smaller caseload and supervises up to 2 Care Managers.  The Senior CM is ultimately responsible for the overall provision and coordination of services to assigned caseload, as well as oversee the service delivery and training of their supervisees.   

 

Primary Duties and Responsibilities:

  • Provide regular supervision to assigned Care Managers – review of cases, assist with training & development needs, compliance issues, guidance on caseload needs
  • Co-sign progress notes, comprehensive assessments and plan of care issues of supervisees’ caseloads
  • Review CANS-NY completed by supervisee prior to finalization
  • Participate in Administrative Team meetings
  • Assist in running of monthly team and staff meetings
  • All duties and responsibilities of the Care Manager position for assigned caseload and oversee these activities of supervisees
  • Obtaining or updating consents as needed
  • Maintain required contacts per member acuity
  • Complete all required assessments: crisis plans, comprehensive assessments, monthly billing questionnaires, CANS-NY,
  • Timely documentation of all activity with an encounter and corresponding progress note.
  • Development of Individualized Plan of Care with participation of the member and member’s family
  • Coordination of services for members
  • Conduct interdisciplinary team meetings on members’ care teams
  • Collaborate with providers 
  • Provide referrals & linkages to and advocate for members as needed
  • Provide informal supports to member and family
  • Provide transitional supports and 
  • Attends and participates in ongoing staff development trainings to enhance skills needed to effectively meet the demands of the Care Manager position
  • Ensure all of the above responsibilities provided by supervisee are understood, met and best practices followed
  • Assist QI or Director with reports as needed
  • Other duties as assigned

 

Qualifications and Competencies:

Education and/or credential requirements are determined by children’s acuity level and requires one or more of the following:

  • Bachelors of Arts or Science with four years’ experience OR
  • A Master’s Degree in related field (License preferred) and two years’ experience
  • Bilingual is a plus 

 

The candidate must possess the following Experience:

  • Demonstrated competency in providing Health Home Care Management services
  • Relevant expertise and experience in serving children and families in child welfare, developmental disabilities, mental health, healthcare and/or other systems as well as those receiving preventive services

 

In addition the candidate must demonstrate the following Competencies:

  • Excellent writing, communication, and organizational skills
  • A passion for serving children and families with special needs
  • Ability to travel to community-based agency offices, participant’s communities, and homes within assigned catchment area 
  • Ability to receive feedback to professionally grow and/or improve
  • Proven self-management abilities, including meeting deadlines, ensuring compliance with agency policy and procedures, and overseeing complete and timely maintenance of agency records, in accordance with contractual requirements

Physical Requirements: Must have the ability to complete all standard administrative and support tasks including but not limited to climbing up and down stairs, operation of computers/phones/fax/printer/copy machine, and the ability to lift boxes, furniture, and equipment up to 30lbs.
