Sheltering Arms is seeking a Family Support Worker to provide case management and parent education services to prenatal and parenting women with children 0-5.

Essential Functions:

  • Initiate and maintain regular home visits (as per HFNY guidelines and standards) with families primarily in their homes (some evenings and weekends as necessary)
  • Establish a trusting relationship with families
  • Provide prenatal and other health education
  • Assist families to strengthen parent-child relationships
  • Assist parents to improve their skills to optimize the home environment for the child’s well-being and safety
  • Assist families in making and attending health and human service appointments, including activities related to employment and educational goals
  • Provide timely documentation of all home visiting activities, including information gathered on health and development, parent-child relationship, maternal life course including documentation, completes all relevant data and maintains records accordingly; Case Notes, Home Visit Narratives and other program documentation must be maintained as per agency &program guidelines.
  • Responsible for working client holiday events and program activities 
  • Participates in outreach, and other efforts to educate the community about the program
  • Other duties as assigned

 

Qualifications:

  • Associate’s Degree preferred but the minimum requirement is a high school diploma/GED
  • Experience working with high-risk families in a community setting
  • Have knowledge of infant and child development
  • Ability to be non-judgmental and establish trusting relationships; good problem solving, listening and practical skills; demonstrates maturity, self awareness and emotional well-being;  shows respect and sensitivity with individuals of differing cultural, racial, sexual or ethnic identities; some knowledge of community resources.
  • Open to working evening hours and weekends when necessary
  • Fluent in Spanish
