Family Support and Resource Specialist

The Family Support and Resource Specialist is responsible for providing case management and parent education services to prenatal and parenting women with children 0-5.  They will also assess families for maternal child health services using the Parent Survey.

Essential Functions:

  • Assess parents for enrollment into Home Visiting using the Parent Survey which assesses for domestic violence, family stressors, bonding and attachment issues and other risk factors associated with child abuse and neglect
  • Participate in street and community outreach, community presentations and other efforts to educate the community about the program
  • Conduct regular home visits (as per HFNY guidelines and standards) with families (some evenings and weekends required)
  • Provide parenting education and reflection to assist families in strengthening parent-child relationships and reduce the risk of child abuse and neglect
  • Promote family health and safety by providing education around safe sleep, health hazards, lead, immunizations and well-baby visits etc as well as referring and accompanying parents to health and human service appointments
  • Conduct data entry electronically using the Management Information System (MIS)
  • Provide community referrals and accompany families to appointment as needed
  • Assist families in creating meaningful goals which are reviewed at regular intervals
  • Provide timely documentation of all home visiting activities, including information gathered on health and development, parent-child relationship, and family functioning; this includes data entry responsibilities in an electronic database
  • Organize and execute tasks at client/program activities and events, which may be held in evenings or on weekends and may require some moderate lifting of items, creation of flyers and other tasks related to the event
  • Participate in the sorting, organizing and storage of goods donated or purchased for families
  • Complete all orientation and wrap around training as well as a minimum of 30 hours of training per year which may require overnight or weekend travel upstate and out of state
  • Provide case coverage when other staff may be incapacitated, on extended leave or during vacancies
  • Other duties as assigned


  • High School Diploma or GED required, Associate’s Degree preferred
  • Experience working with high-risk families in a community setting
  • Have knowledge of infant and child development
  • Ability to be non-judgmental and establish trusting relationships; good problem solving, listening and practical skills; demonstrates maturity, self awareness and emotional well-being;  shows respect and sensitivity with individuals of differing cultural, racial, sexual or ethnic identities; some knowledge of community resources.
  • Open to working evening hours and weekends when necessary
  • Bi-lingual (Spanish) required
